Change Request Form

Dear Parents/Guardians and Students:

We hope you and your families are well and we thank you for your continued support through our school reopening. As shared earlier this school year, families will have a second opportunity to request a change in their school program model. Information gathered through this Model Change Request Form will support planning for our Semester 2 Adaptive Face-to-Face and Virtual Models. We are making every effort to ensure that both in-person and virtual learning offer students safe, inclusive, supportive and engaging learning environments.

Through this Model Change Request Form, parents/guardians of secondary school students may request a change from Adaptive Face-to-Face learning to Virtual learning, or from Virtual learning to Adaptive Face-to-Face. Information about both learning models can be found on the Board website at www.yrdsb.ca/schools/school-reopening.

On Monday, November 9, Parents/Guardians of secondary school students will be able to access the School Model Change Request Form through their child’s Gapps account. Parents/guardians will work with their child to complete the Google form and once submitted, a copy of the response will be available in the student's Gapps email account.

Only families of secondary students requesting a school model change will need to complete this form. The form must be completed by Monday, November 16, 2020 at 8 a.m. to be considered. If you do not wish to change models, there is no action needed on your part.

After the School Model Change Request Form is closed, all families will receive an email to confirm their Semester 2 placement request by Wednesday, November 18, 2020. Students who are moving to a new school model can expect to start in the new model along with all students at the beginning of Semester 2 on February 2, 2021. Schools will complete student timetabling in January. As a result of program changes to support these school models, not all semester 2 secondary courses or program pathway options may be available in both models or in all schools. The Board will communicate any program and placement decisions as early as possible to support family and school planning. The home school Guidance staff will work with students and families to support timetable changes in both the Face-to-Face and Virtual models if needed.

As we indicated previously, moving between school models can only be considered at certain points in the school year. Beyond this opportunity, there will be no further opportunities to consider changes between the secondary school models this school year.
